Hot Flashes
Estrogen therapy is the most effective treatment for hot flashes

Bioidentical hormone therapy to restore energy, balance and vitality, for women and men.
The problem and the plan
Hormones act as messengers for nearly every system in the body. In women, estrogen and progesterone fall during perimenopause and menopause, which can bring hot flashes, night sweats, poor sleep, vaginal dryness, joint aches and mood changes. In men, testosterone slowly declines with age and with some health conditions. Low levels can drain energy, muscle, sex drive and focus. Many people are told these changes are just part of getting older.
Hormone replacement therapy starts with answers, not guesses. We match your symptoms to your lab results, then build a plan that uses the lowest effective dose. Women may receive estrogen, often with progesterone. Men with confirmed low levels may be guided toward testosterone therapy for men. We recheck labs and symptoms often and change the plan as your body responds.
Good to Know
Bioidentical hormones, explained simply
Hormone replacement therapy (HRT) replaces hormones your body no longer makes in healthy amounts. For women, this usually means estrogen, plus progesterone if you have a uterus. Bioidentical hormones match the structure of the hormones your body makes. Several are FDA approved, such as estradiol and micronized progesterone. Custom compounded versions are not FDA approved.
Hormones can be given as patches, gels, creams, pills or injections. Many patients notice better sleep and fewer hot flashes within weeks. Mood, energy and comfort often keep improving over a few months.

What is normal and what to report
Early side effects are usually mild and often fade as your dose settles. They can include breast tenderness, bloating, headaches, mood shifts, spotting or skin irritation where a patch or gel is applied. Men on testosterone can notice acne, fluid retention or a rise in red blood cell count.
Serious risks are less common but important. Estrogen can raise the risk of blood clots and stroke, especially in pill form and in women over 60. Estrogen without progesterone raises the risk of uterine cancer if you have a uterus. Combined therapy may slightly raise breast cancer risk with long use.
MedlinePlus explains these risks further. Call us right away about chest pain, leg swelling, sudden headache or unusual bleeding.

Yes. Hormone therapy is available end to end by telemedicine for eligible patients, including consultation, lab review, dosing and follow-up. Telemedicine visits are offered Monday and Tuesday evenings and on Thursday and Friday. In-person visits at our Gulf Breeze office are also available on Thursday and Friday.
Some are and some are not. FDA-approved bioidentical options include estradiol patches, gels and pills and micronized progesterone. Custom compounded hormones are made by pharmacies and are not FDA approved, so their strength can vary. The Menopause Society recommends FDA-approved products for most women. Dr. Boler explains both options honestly.
For most healthy women under 60 or within 10 years of menopause, the benefits of hormone therapy usually outweigh the risks. Risks such as blood clots, stroke and breast cancer depend on age, dose, route and personal history. Patches and gels carry a lower clot risk than pills. Dr. Boler screens you carefully before starting.
Many patients notice changes within two to six weeks. Hot flashes and night sweats often ease first, followed by better sleep. Mood, energy and comfort usually keep improving over about three months as your dose is adjusted. Results vary, and we recheck your labs and symptoms to fine-tune your plan.
Most patients have labs and symptoms rechecked within about three months of starting, then at regular intervals once the dose is stable. Women on estrogen usually need yearly reviews and routine breast and pelvic screening. Men need closer monitoring of blood counts and prostate health. Follow-up visits can be done by telemedicine.
No. We offer hormone optimization for women and men. Women usually seek care for perimenopause and menopause symptoms. Men with symptoms and confirmed low testosterone on lab testing may benefit from testosterone therapy, which has its own page. Dr. Boler reviews your symptoms and labs before recommending any hormone.
HRT is the broader term for replacing hormones that have declined, most often estrogen and progesterone in women. Testosterone replacement therapy focuses on men with confirmed low testosterone. Both need lab testing and monitoring. Dr. Boler can explain which path fits your symptoms, your labs and your goals.
Hormone therapy is usually not advised if you have had breast or endometrial cancer, a blood clot, stroke or heart attack, active liver disease or unexplained vaginal bleeding. Pregnancy is also a reason to avoid it. Dr. Boler reviews your full history and will suggest non-hormone options if HRT is not safe for you.
