Low Energy
Daily fatigue that rest and coffee do not fix

Testosterone therapy for energy, strength and men's health, guided by your labs.
The problem and the plan
Testosterone helps men build muscle, keep bones strong, make red blood cells and maintain sex drive and energy. Levels drop slowly with age and can fall faster with obesity, diabetes, sleep apnea, some medicines and injury. Low testosterone, also called hypogonadism, can cause fatigue, low libido, erectile problems, lost muscle, more belly fat and a low mood. Many men assume these changes are normal and never get tested.
Testosterone therapy starts with a clear diagnosis. We confirm low levels with early morning blood tests on two separate days, look for treatable causes and review your goals. If TRT fits, Dr. Boler sets a dose and schedules regular lab checks. Women and men who want broader hormone care can explore hormone replacement therapy at our practice.
Good to Know
Restoring a normal testosterone range
Testosterone replacement therapy (TRT) is a prescription treatment that raises low testosterone back into a normal range for men with hypogonadism. FDA-approved testosterone products are approved for men with low levels caused by a medical condition. The Endocrine Society guideline advises treating only men with both symptoms and consistently low lab results.
Testosterone can be given as injections, gels or other forms. Many men notice better energy, mood and libido within three to six weeks. Muscle strength and body composition change more slowly, often over several months.

Your options side by side

| Option | How It Works | Dosing | Effect on Fertility | Monitoring | Main Drawback | Best For |
|---|---|---|---|---|---|---|
| Testosterone injections | Testosterone delivered into muscle or under the skin | Every 1 to 2 weeks, or longer | Lowers sperm production | Hormone levels, hematocrit, PSA | Peaks and dips between doses | Men who prefer fewer doses |
| Testosterone gels | Testosterone absorbed through the skin | Daily | Lowers sperm production | Hormone levels, hematocrit, PSA | Can transfer to partners or children by touch | Men who prefer steady daily levels |
| Non-testosterone medicines | Prompt the body to make more of its own testosterone | Varies | Usually preserves it | Hormone levels and symptoms | Often off-label use | Men who want to keep fertility |
| Lifestyle changes | Weight loss, sleep, strength training and diet | Daily habits | No harm | Self-tracking and labs | Slower and smaller change | Borderline levels linked to weight or sleep |
TRT suits men who have both symptoms and lab-confirmed low testosterone. Our article on low testosterone symptoms and when to get tested is a helpful first step. The American Urological Association also publishes guidance on testosterone deficiency.

From first visit to follow-up
Dr. Boler reviews your symptoms, health history and goals, in person or by telemedicine.
We order two early morning testosterone tests plus blood count, PSA and other safety labs.
If TRT fits, Dr. Boler chooses the form and dose and explains fertility and safety tradeoffs.
We recheck testosterone, hematocrit and PSA at about 3 to 6 months, then at least yearly.
What is normal and what to report
Common side effects include acne, oily skin, fluid retention, breast tenderness and skin irritation where gel is applied. Gels can pass to partners or children through skin contact.
Testosterone raises red blood cell production, so hematocrit can climb too high and thicken the blood. It also lowers sperm production and can shrink the testicles. TRT can worsen untreated sleep apnea and may speed the growth of an existing prostate cancer, which is why we check PSA.
In the large TRAVERSE trial, testosterone did not raise heart attack or stroke risk, but atrial fibrillation and lung blood clots were more common. Call us right away about chest pain, leg swelling, trouble breathing or trouble urinating.

Low testosterone is confirmed with blood tests, not symptoms alone. Guidelines recommend two early morning tests on separate days, since levels change during the day. Common symptoms include fatigue, low sex drive, erectile problems, lost muscle and low mood. Our guide to low testosterone symptoms explains the warning signs in more detail.
Many men notice better sex drive, mood and energy within three to six weeks. Changes in muscle mass, strength and body fat take longer, often three to six months or more. Bone density improves over a year or longer. Results vary, and we adjust your dose based on follow-up labs and how you feel.
TRT requires regular lab checks for safety. We check testosterone levels and hematocrit, a measure of red blood cells, about three to six months after starting and then at least yearly. Men in the recommended age range also have PSA checks for prostate health. Thick blood from a high hematocrit can raise clot risk, so we adjust the dose if needed.
Yes. Testosterone therapy lowers sperm production and can cause infertility, sometimes for months after stopping. It is not a good choice for men trying to father a child soon. If you want to keep fertility, tell Dr. Boler at your first visit so he can discuss other options that may protect sperm production.
For most men with confirmed low testosterone, a large 2023 trial found TRT did not raise the risk of heart attack or stroke compared with placebo. The same trial saw more atrial fibrillation, kidney injury and blood clots in the lung. Dr. Boler reviews your heart history and monitors you closely during treatment.
TRT focuses on men with confirmed low testosterone. HRT is the broader term for replacing hormones that have declined, most often estrogen and progesterone in women during menopause. Both need lab testing and follow-up. Dr. Boler can explain which approach fits your symptoms, lab results and goals.
TRT is usually not advised for men with prostate or breast cancer, a high hematocrit, untreated severe sleep apnea, poorly controlled heart failure or a recent heart attack or stroke. It is also not advised for men who want to father a child soon. Dr. Boler reviews your history and labs to make sure treatment is safe.
